Please download the registration package with all the necessary forms that we require. To ensure legibility and accuracy of information, we ask that your information be typed directly into the forms before printing and signing in the appropriate places.
  
Please return the forms with a photocopy of your child’s Health Insurance or Medicare card (front & back) to H₂O Adventures.
  
Completed forms may be faxed by calling first to the H₂O office or mailed.
